Why use FormCLIC?

To reduce paperwork, to avoid errors in re-copying information and to save time, ATQ has developed an electronic report form to declare:

1. Animal births and tag installations;
2. Tag activation for animals coming from outside Quebec;
3. Tag replacement;
4. Reception of animals originating from within Quebec;
5. Animals being shipped out of Quebec;
6. Animal deaths.

How to install the software on my computer

1. Downloading FormCLIC is done in six steps. To start the download, click on the version corresponding to the field of production you wish to install:
   1. Click here for download SLAUGTHERHOUSE application
2. Save the file to your computer Desktop;
3. Now, double click on the file that appeared on your Desktop;
4. Follow the installation instructions that appear on your screen;
5. Then, delete the file from your Desktop, once installation is complete;
6. That's it. You are now ready to use FormCLIC.

Using the software

Initial startup of the FormCLIC software on my computer

The first time you start FormCLIC, you must register your business information. You must have your Agri-Traçabilité Québec stakeholder number and your premises numbers.

• Click on (The first icon at the top left of the screen);
• Key in valid information about your business;
• Press on

You must register you business information only one time; they will be kept for further use of FormCLIC

Once the software installed, how do I use it?

Download the User's Guide

You can complete section 5.1 (ATQ Identifier number) in the Events tab by importing the data from an RFID reader. Of course, you must first have read animal tags with the reader.

• In the tab Slaughtering Declaration, click on the Reader menu;
• Select Import identifiers from the reader in the Slaughtering declaration tab.

You can now empty the reader memory when you are finished filling your form.

• Click on the Reader menu;
• Select Empty the reader.

FFormCLIC is compatible with the readers of  Syscan International,  Allflex Canada and  Gallagher .

Advanced use of FormCLIC
